SOCIAL MEDIA SHOWCASE - Texas Tech University Health Science Center of the Permian Basin Our didactics are every Monday 1-5p, and Tuesday 12-2p which consist of a combination of lectures from attendings, residents, subspecialty attendings, research, guest lecturers, and simulations. Didactics are protected time for residents and the attending will carry the call phone. Our simulations include PPH management, normal vaginal and operative delivery, management of pre-eclampsia/eclampsia, laparoscopic/hysteroscopic skills, breech deliveries, LEEP, etc.. These occur at least monthly with combination of lectures and hands-on simulation. Also, wellness has evolved as an essential core curriculum for many residencies and we are no different. Fortunately, we have Dr. Hutton coordinating our curriculum and we have had so much fun and team-building with activities such as Escape Rooms, group exercise/yoga sessions, bowling, and resources available to aid with our mental health! We are required to have a research project and there has been plenty of opportunities to form or join one of the many projects going on! SOCIAL MEDIA SHOWCASE - Texas Tech University Health Science Center of the Permian Basin We are fortunate to be affiliated with all the hospitals in Permian Basin! We spend majority of our time at Medical Center Hospital (MCH ) in Odessa, but also able to participate in gyn surgeries and deliveries at Odessa Regional Medical Center (ORMC ). Starting at the end of intern year, we start to work at Midland Memorial Hospital (MMH ) for deliveries and gyn surgery as well. All the facilities have Da Vinci Xi robot system, and we are able to start our robotic training as early as intern year! We become primary surgeons starting intern year with primary c/s and initially uncomplicated laparoscopic/hysteroscopic surgeries but will rapidly evolve into more complicated cases as intern year goes on. Our upper levels have the ability to attend/assist cases with our lower levels as teaching assists! Not only do we perform surgeries/deliveries with our attendings but we also work with a multitude of community private physicians as well. The opportunity to learn variety ways of management is abundant and it helps hone our personal styles of management. Susan Smith is a dedicated liaison who serves as somebody to chat with, get coping tools from, and who will help coordinate further referral if needed (psychology, family counseling, etc ). Dr. Nakad is one of our MFM's and is always looking for ways to get residents more hands-on experience. He organizes simulations on Wednesday mornings for our OB team, in addition to running the diagnostics in obstetrics and gynecology (DOG ) conference every Friday AM. ⠀ ⠀ Dr. Nakad also spearheaded securing funding to get us SonoSim simulators! They simulate ultrasound experience and can be done from your personal laptop. Picture of a placenta, also called the "tree of life"! . The placenta is a temporary organ that connects the developing fetus via the umbilical cord to the uterine wall to allow nutrient uptake, thermo-regulation, waste elimination, and gas exchange via the mother's blood supply; to fight against internal infection; and to produce hormones which support  #pregnancy . . If you  #deliver  your baby vaginally, you'll also deliver the placenta vaginally — during what's known as the third stage of labor. After you give birth, you'll continue to have mild contractions. Your health care provider might give you a medication called  #oxytocin  to continue uterine contractions and reduce postpartum bleeding. You might also massage your lower abdomen to encourage your uterus to contract and expel the placenta. If you have a C-section, your health care provider will remove the placenta from your uterus during the procedure. The placenta will then be examined to make sure it's intact. SWEET SIXTEEN! This week we’re celebrating the sixteenth anniversary of SRM serving the families of the Pacific Northwest. Back in 2004 our founding physicians Dr. Nancy Klein, Dr. Paul Lin, Dr. Michael Soules and Dr. Angela Thyer opened our first clinic located on Westlake Ave at South Lake Union. We then opened our SRM doors in Bellevue, then Tacoma in 2007, Kirkland in 2010, Spokane in 2013, and our newest location in Everett in 2019! Every year, we gather all staff from each location to celebrate all that we’ve accomplished and recognize our staff for their commitment and dedicated to providing the best patient care experience and results in the PNW. Like everything else in 2020, this years celebration was a little different but valued just as much. In Spokane, our team gathered (masked & 6 ft. apart ) to join with Seattle folks via Teams to hear from our Executive Director Brad Senstra and an SRM patient. We recognized our 3, 5, 10, and 15 year employees as well as Employee of the Year nominations and cheesy “Velveta” awards. Finally, we enjoyed virtual fun events including escape rooms & trivia. 🔆 W E L L N E S S 🔆 We had so much fun enjoying the beautiful weather last night for our Oberun 5K/dog parade 😂 Our initial run was canceled due to ⛈ so shout out to our Wellness Chief @haileymarn who worked so hard to find another date (It’s difficult to coordinate resident schedules with the amazing but sometimes moody Michigan weather! ) Thank you KOC for the Jimmy John’s and refreshments afterwards— always keeping us well fed 👌🏼 Just one of the many ways we try to incorporate and encourage wellness in residency. A few more examples are team building activities (pasta making at Zingerman’s, Whirly Ball, Knocker Ball ) and protected time during Grand Rounds to go to appointments or get an oil change. I N T E R N Y E A R As I’m a few days from starting my role as chief resident, I can’t believe how far the woman in this picture has come. Intern year, I was so nervous and scared. I had no idea what I was doing and looking back this is the advice I would have given myself. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ #wishiknewinternyear ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 1. It's okay to not know something. It's okay to not be perfect. The most important thing is to take excellent care of your patients. Focus on that. 2. Residency is hard, but it's designed that way. You will feel overhwhelmed at times and feel like you're biting off more than you can handle. But that's how you learn and how you grow. Embrace it. 3. Celebrate the small victories whether it's delivering your first baby or remembering to put in all of your orders without being reminded. Add to your bucket to keep it from becoming empty. 4. Don't forget about you. You can't take care of patients well without first taking care of yourself. Exercise. Eat healthy. Do things outside of medicine that make you feel well.  Residency is a marathon and you need to train like it. 5. Residency goes fast. Soak up every opportunity to learn and grow because before you know it you'll be a chief mentoring a new set of interns.

I looked back and found this oldie: my first picture taken as a resident at Yale. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ I thought I’d look back at how far I’ve come since today I applied for maternal fetal medicine fellowship, the next chapter in my career after residency! ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ For those that don’t know, maternal fetal medicine physicians take care of women with complicated pregnancies. Women with diabetes, heart disease, autoimmune diseases, prior poor pregnancy outcomes and fetal anomalies are just some examples of patients I’ll be taking care of. Since medical school, I’ve loved pregnancy and all of its intricacies. After being pregnant, it solidified my love for this field. The vulnerability in pregnancy is like no other. These women especially have complex problems that are difficult to understand and often leave them with high risk but high reward pregnancies. I can’t wait to be their doctor who sits with them and explains to them exactly what’s going on and in ways they understand, the doctor who holds their hand during some of the hardest experiences they’ll ever have to endure. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ If there’s anything I’ve learned about these women is that they will do anything for their children and they deserve a provider who’ll do anything for them. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ I just can’t wait!

Qui sono solo un #visitor cioè purtroppo, posso solo osservare, senza toccare. Ma questo come al solito non mi ha fermato, ho ascoltato, ho osservato, ho fatto mio più cose possibili. Ho chiesto ad #infermieri ad #dottori a #farmacisti a praticamente tutti, informazioni ℹ️, protocolli, #guideline everything. E ho tratto le mie conclusioni sul modo di lavorare, sul modo di rapportarsi al paziente ecc ecc, usciamo una serie di video sul mio canale YouTube Aboutpeppe893 ( che vi invito a seguire ) molto interessanti sempre a tema #medicina qui dagli #statiuniti e in particolare modo da #losangeles . Ritmi ingresso, ma di sicuro uno spoiler ve lo posso fare, qui tutto funziona alla perfezione. Gli orari di lavoro sono massacranti circa 12h al giorno 7 Am to 7:30 pm ma va bene così. Stanchezza tanta. SECONDO ME IL SISTEMA SANITARIO AMERICANO IN TERMINI DI EFFICIENZA DEL SERVIZIO È 100 VOLTE DI QUELLO ITALIANO, SECONDO VOI? This week we had our 1 month wedding anniversary but neither of us remembered 🤪 because I was on my 7th of 8 night shifts over 9 days. Then we had one night together and Michael had a night shift. 🌚 . Welp, this is our life for the next few years as an OB Laborist and Peds ICU doc! Never a dull moment for shift work doctors 🥴🧛🏻‍♀️👩🏼‍⚕️👨🏽‍⚕️. . After this guy wakes up we’re going to get REAL clothes on and celebrate his graduation from Pediatric Cardiology fellowship! 👨🏽‍🎓 I’m so proud of the hard work he’s done over 3 years, and even more proud he’s one to the few people in the whole country who is pursuing TRIPLE board certification by also doing a Pediatric Critical Care fellowship. . Happy Sunday!!!⠀ ⠀ I love spring!⠀ ⠀ (Except for my allergies. This is not a time to be sneezing and congested all the time, right? ) 🤔⠀ ⠀ This week I will be posting about miscarriages. As a fertility specialist, I spend a large part of my time counselling women about this particular situation⠀ ⠀ I have 4 pieces about different aspects of miscarriages that will be posted throughout the week.⠀ ⠀ On Thursday, Dr. Wais and I will do our third IG live episode, and the topic is going to be miscarriages. We will be taking questions as always. Will post more on that later⠀ ⠀ I have had my own personal experience with miscarriages ( even a euploid embryo ) and I have thought long and hard about how to talk about this with you all.⠀ ⠀ My goal is always two-fold: education and inspiration⠀ ⠀ I want you to have the knowledge. But even more than that I want you to know that you are not alone and you CAN DO THIS. This little guy is everything. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ When I was first pregnant, I was so excited to document my pregnancy and share those experiences with all of you. I mean I was a pregnant OBGYN experiencing all the things I teach my patients about. I had so much to say (round ligament pain is real and peeing five times a night is a norm in the third trimester ). If you look back at my feed though, I only posted a handful of times, which definitely stemmed from our experience in early pregnancy. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Chris and I had genetic screening in early pregnancy called cell free DNA. It came back indeterminate. What does that mean, right? It means a “no call” test or that they couldn’t interpret the result, which happens for a variety of reasons. Low fetal fraction meaning the fraction of cells from the pregnancy were too low to make a determination or there’s a specific genetic component making the test unable to be resulted. People with these results are thought to have a higher risk of genetic abnormalities and therefore it’s recommended to consider further studies. All of a sudden our perfect pregnancy was full of questions and what if’s? What if something was wrong with our baby? What if we do further testing and something happens? What if we go through all this anxiety for nothing? ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ I remember feeling a lot of the happiness from the first trimester melt away from the uncertainty. When people congratulated me, I would fake smile and later cry. We ended up having a CVS, a procedure that takes a part of the placenta to look for genetic abnormalities. After a few weeks, we had our answer. Everything seemed fine, and our pregnancy seemed normal. I couldn’t shake that feeling that had developed. For awhile it felt like something was bound to happen and as an obgyn I’m all too aware of the unimaginable things that can happen. I feel for you mamas out there that can relate.⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Now that he’s in my arms and I’ve had the opportunity to reflect on my pregnancy, delivery and postpartum period, I can finally say I’ve shaken that feeling and I’m ready to be the support to others that I initially set out to be!
